Creating and adapting forms is efficient and highly visual with the state-of-the-art Form Designer embedded within MyLearningPlan OASYS. The Form Designer allows specified users to create any form that is needed to support the evaluation process. The technology allows the user to easily drag and drop form sections to the on-screen canvas, and customize the properties as needed, in order to see the form being created in real-time.
Any rubric may be incorporated into any form design, and form designers can easily preview their work and save forms as drafts for later completion. MyLearningPlan OASYS enables all forms to be viewed as a PDF file or printed.

We also have a library of template forms that have been designed based on the latest, proven evidence and best practices, which are available for immediate use or customization. In addition, MyLearningPlan OASYS provides access to electronic state-specific forms to ensure that required fields are included.
