Evaluation Forms

 

Evaluation forms are a critical component in assessing the learning that educators gain from professional development. With the Evaluation Tool, you can collect feedback and reflections, pre- and post- data, and analyze the effectiveness of professional learning. Evaluation forms may be renamed “Reflections,” “Summaries” or any other title you’d like. Develop your own questions, including Likert Scales, single or multi-select questions and free text. View results in graph and chart formats. You can even mandate completion of evaluation forms prior to the granting of final credit.

Application & Impact Forms

 

Modifiable Application and Impact forms are available to help collaboratively establish expectations and assessment criteria for transfer of new knowledge  and skills into classroom practice, and capture evidence of the impacts on student outcomes. These forms may include a variety of fields, including expected changes in teacher practice, anticipated student outcomes, assessment measures, forms of evidence, and both baseline and resulting student data.

Copy or set up your own Log form to serve as a reflective practice journal. On a regular basis, educators can easily note information about how they are applying knowledge and changing their practice, and the effects on their students. At the end of the established period of time, users submit their collection of logs for review and approval.
